Why Does My Child Need A Minor Travel Permission Slip?
Any time a minor is traveling without a parent or legal guardian, they will need a signed and notarized Minor Travel Consent Form.
The airport approved Minor Travel Consent Form allows an unaccompanying parent to affirm to the child’s travel plans, but more importantly, it helps deter human trafficking and kidnapping.
The Minor Travel Consent Form, also known as a Minor Travel Permission Slip, is a two page document that requires a child’s parent(s) to attest to the child’s travel plans and agenda. Throughout the documents the parent(s) will be instructed to provide not only their name(s), but also the name of the child that will be traveling; along with the child’s date and place of birth, and passport information. In addition to the aforementioned details, the form also requires that the parent(s) specify whether the child will be traveling by themselves, or with another person or organization - such as a church group or a cohort from the child’s school. In the event that the child will be traveling with another individual or organization, you must list the individual’s / organization’s name, and if applicable, their relationship to the child and the pertinent details from their passport (Passport Number, Country of Issuance, Date of Issuance, and Date of Expiration). Lastly, the form instructs you to list the details of the child’s travel plans - where the minor will be traveling to, when they’ll be leaving, and when they’re intended to return. To complete the document, you as the child’s parent(s) must sign and date the form in the presence of a notary.
Once the Minor Travel Consent Form is completed, it is ready to be notarized. In order to notarize the documents, you will need to provide the the notary with the completed Minor Travel Consent Form and at least one form of ID; some examples of acceptable forms ID include: a U.S. or foreign passport, a state issued driver license, or a state / government issued non-drivers ID. The notary will utilize the provided form(s) of ID to verify the parent(‘s) identity and carry out to the notarization process.
